We have a basic financial policy and a supplemental financial policy for patients who have Premera Insurance. In addition, we accept cash, check, Visa, and MasterCard. Basic Financial Policy
For restorative treatment appointments requiring one hour or longer, we ask for full payment at the time of scheduling. Your insurance company will reimburse you directly. For your convenience, we will administer your claims and provide the insurance company with all the necessary documentation and radiographs needed to maximize your insurance coverage. 
Supplemental Financial Policy (Premera Insurance)
We ask that you pay your estimated portion at the time of scheduling, and that you pay your portion at each appointment. For your convenience, we will submit all your claims directly to Premera. We will administer your claims and provide the insurance company with all the necessary documentation and radiographs needed to maximize your insurance coverage. However, you will be responsible for any balance remaining that is not covered after Premera has made its payment.  Our Smile Artistry Team is proud of the dentistry we provide and we are confident in the durability of our treatment. Our ultimate goal is to provide you with an everlasting, healthy smile. Part of our approach is to show you how to prevent future dental problems. The long-term success of your dental treatment really depends upon how well you take care of your teeth. We want to encourage preventative care. This will save you time and unnecessary expense in the long term. Regular professional exams, cleanings, and fluoride treatments are an essential part of dental healthcare. Your individual condition depends on the frequency of those continued care visits and the products we recommend for you. With that in mind, we are very pleased to offer you the following dental treatment warranty at our cosmetic dentistry practice in Redmond, Washington. CROWNS, BRIDGES, INLAYS, ONLAYS, AND PORCELAIN VENEERS If these porcelain restorations fracture with normal use during a period of a full five years from the date of their initial placement, we will replace or repair them at no additional charge. COMPOSITE BONDING OR FILLINGS For composite (tooth colored) bonding or filling restorations, we will replace or repair them in the event of failure for a period of two years from the date of their initial placement. DENTAL SEALANTS Sealants are plastic coatings placed on the chewing surfaces of the teeth to prevent tooth decay in the pits and grooves. These are the most common areas to get cavities. Flossing and the use of fluoride after their initial placement will help prevent tooth decay between teeth. We will repair or replace any of our sealants that fracture or break for a period of 18 months from the date of their initial placement. TEETH WHITENING
Immediately after your teeth whitening treatment, if your measured shade of whitening does not increase by at least two shades on the anterior teeth, we will either refund your teeth whitening fee or provide another series of treatments.
CONDITIONS
To keep this warranty valid, you must: 1) keep your prescribed regular continuing care appointments (with no appointment varying more than 30 days of our recommended schedule); 2) maintain your account in good standing; and 3) have all ideal recommended dental treatment(s) performed by Dr. Karen McNeill (with the exception of treatment specifically recommended elsewhere). This warranty does not cover procedures not mentioned above, including root canal therapy, gum desensitization, and night guards; nor does it cover damage to teeth or dental prosthesis caused by trauma, accidents, abuse, neglect, or improper use (e.g., clenching, grinding, chewing ice, or biting on non-food items).
